Your First Office Visit

Care Associates of Paris as pleasant and comfortable as
possible. Here are a few tips that will help us serve you better.
What to expect
Our office will seem familiar, unless you have never visited a doctor’s

you may not have seen model trains in your doctor's waiting room before!)
and more examining rooms than you have seen before, but you will receive
helpful, individual attention from the moment you check in at the reception
Please dress comfortably for your office visit and, if possible,
avoid wearing perfume or after shave.
On your first visit, we will ask you for some information about yourself
and your insurance carrier. We will want to make a copy of your insurance
card. You will also be asked to complete a New Patient Registration Form.

and bringing it with you.)
You will also be expected to pay your insurance co-pay at the time of
this and all other office visits. We will file with your insurance company
for the remaining charges associated with your visit. If you do not have
insurance (or we can’t verify your insurance) you will be asked
to pay for your office visit and medical services at the time of your

What to bring with you
To make your initial registration go smoothly and to assist our medical
staff to provide you with the care you need, please bring the following
with you for your first visit:
Note: If this first office visit is for a child, please bring medical
history and medication information for that child. We will still need
insurance information for the child’s parent or guardian.
